- Patient’s information will remain confidential, except when it becomes necessary to provide certain services, or make sure that all administrative manners pertaining your well being are being managed accordingly. This includes sharing your information to other health providers, laboratories, and insurance companies when necessary and appropriate for your care.
- It is our policy to use the patient’s email as means of communication. This means you could receive medical service-specific notifications via email.
- Your personal and clinical information will not be used for marketing purposes of any nature.
- This application grants you access to your personal health record, with the data specific to our client laboratories.
- We can modify these policies as needed to provide better quality of service to both, our patient and provider communities.
- You have a right to request restrictions over your medical information with the specific laboratories or medical providers you share information with through our services.
- Laboratories utilizing our services could handle your data in any manner they require to offer their services. Syndeo does not control how the laboratory environments utilize this information once captured.
- The device’s camera and/or picture gallery service consumption is intended to have images of the following:
- Main User’s Profile
- User’s medical record photo (optional)
- User’s medical ID (optional)
- User’s insurance card picture (optional)
- Family Profiles
- Family member’s profile photo (optional)
- Family member’s ID (optional)
- Family member’s insurance card picture (optional)
- Medical orders (optional)
- Order Tests
- Read test kit’s ID barcode (required)
- Submit medical order and other medical information (optional)
- Other than the features mentioned above, the device’s camera is not used in any other area of the app at any point. The only required instance where the user must use the camera is to associate the test kit’s ID in the Covid Test workflow.
